I don't suffer too much but I do suffer from Mail's bizarre handling of Junk mail - essentially it seems to decide on a daily basis what the rules are and that includes putting into junk email addresses in my Address Book. Its so inconsistent I have to check my junk folder as much as my inboxes.

One word: Gmail. Even if you hate the UI, just run your email through it behind the scenes. There's no better spam filter out there.
